We have been seeing a spike in the number of duplicate orders placed within minutes of each other for the last week. It seems to primarily be with users shopping on their phones. There appears to be a delay in processing/ confirming the orders back to the customer so they place a second order. Some have said the items stay in their cart so they don't think the order has gone through.
We always contact the customer in these cases via Etsy convos, but they are notoriously bad about replying back. We almost always have to send a separate email to the email attached to the order to get an answer from them. Even then it is iffy. This is annoying and slows our ability to process and ship their orders.
This definitely appears to be an Etsy issue. Please FIX this.
We are also seeing more duplicate orders. Sometimes I chalk it up to impatience and double clicking buy, but when a 25x repeat buyer has 5 items in her cart and we get an order for only 1 of them and then messages saying "I put in an order and it wants me to put in another. Did you get my order?" (which we did, but only 1 item). I don't know if the other items were still in her cart or if she somehow hit "buy now" and only ordered 1 of the 5 she wanted. Fortunately the buyer messaged me about it so we could address the problem immediately; she said she had this happen with another seller. Unfortunately most people think it's an issue with the seller's shop so they blame us for duplicate orders and Etsy collects yet another sale while we're left holding the bag with an angry buyer.
I now cancel every single duplicate order; if it wasn't an error they can reorder. I see there are a few other threads in the technical issues so I'm copying and pasting this since they don't seem to be getting any interest and needs to be addressed.

same happening here.
So much malfunctions on Etsy that I no longer worry about things like this. I wait 24 hours to ship. If a customer contacts me to cancel I do. If they do not contact me all orders are shipped whether they appear to be a duplicate or not. The order was placed, my only responsibility is to ship what was ordered.
