i have used vista prints and logo works. like both of them. also local print shops, like jiffy print,kinko's, etc. may be able to give you a better price and some graphic help.
vista prints probably has the largest selection of pre-fabs and they are very easy to customize, they run specials constantly, etc. there may even be someone with an etsy shop, who knows? check around and you will find something that strikes you right and most places have pretty comparable prices.
