Verify Bank Account and upload Bank statement- Cant do anything

I have had terrible problems trying to verify an account   There is nothing wrong with the bank details and the shop name and the bank account are exactly the same.  I was asked to upload a bank statement and I couldn't do that.

If you have a better idea, plz let me know. I do not want my store get suspended

How did you make out with this?  Etsy is holding my funds hostage because the upload page isn't working.
