Taxpayer ID Issue on new Etsy Store

I am trying to create an Etsy Store, and it was immediately frozen because my taxpayer ID was unable to be verified. It looks like I entered my full name into the FIRST NAME field only, instead of First Name in the FIRST NAME field and last name in the LAST NAME field. My ID number I entered in is absolutely correct.

It appears I am unable to edit my name because it won't re-verify the existing number because it things it is invalid. This is a very frustrating experience so far. If I am unable to get any kind of resolution soon I may as well just close the store.
