Photo of item on gift receipt

I printed out the standard gift packing slip and it now has the photo of the item on there. I have  gone onto "Customise options" next the the receipt and unchecked the box next to "photo" but it is still there after printing. How do I remove it? I am sure the recipient would rather wait until they opened it to see what was inside...

It looks as if the packing slip has been changed by Etsy unannounced. I even had notes saved that I wanted to add on my packing slip and now that option is gone.

Make your own packing slip. Include the name & address of the recipient, and the seller's name & address. No price. Include any message requested. That's all you need.
