Hi. My star seller score has been compromised since Dec. ‘22 due to slow response rate. This is absolutely incorrect and should be at 100%, as I’ve NEVER responded later than 5min to any message I’ve ever received. Does it make a difference where the response comes from? Such as through the Etsy app or email.

The responses must come via messages. Emails don't count.
Hi Billy, as NanaLetha said; emails don't count. Etsy Messages are what counts (whether thru the app or not). Any time you are not the Initiator of an Etsy Message, you must respond to it or Mark as Spam (I only do this with true Spam. i.e. "Hey, buy my gems"). Messages may appear on different Message strings, and even though you believe you've addressed something; again, if you are not the Initiator of a Message, the same rules would apply regardless of what string the Message appears on.

ETA: This also applies to receiving friendly Messages from other Seller; "How's it going?" for example.
