Has anyone done a GCC that they would be willing to share with us? I'm very visual so just reading what needs to be on there really doesn't help me. I like to see format to get a full understanding of what I need to do.

So if I were to go wholesale, I would need to supply one of these for every bib fabric combo?

Yes, although if your fabrics are exempt from testing you would state that with the reference to the exemption rule. Also you would list your Small Business Registration number for this year which gives you exemptions from some testing rules.
