FTC/CPSIA Combo Label

I can't believe I didn't know about this sooner--I did so much research before opening my shop and somehow did not come across these requirements! SO glad to have found them now so I can make sure I'm in compliance. Thank you so much to all of those who have provided guidance in the team forum--I have learned so much in the last 24 hours about the requirements.

I'm hoping to suffice both the FTC and CPSIA label requirements on one label (I already have a custom tag with my business name on my blankets, so this would be in addition to that one. I make baby blankets and burp cloths from cotton flannel, polyester minky, and thread (no other embellishments or decorations). This is what I've come up for my additional label:

Front:
cavucreations.etsy.com
Anacortes, Washington
Handmade in USA
of Imported Fabrics
13JFMAMJJASOND14

Back:
54% Polyester
46% Cotton
Wash Warm, Perm Press
Dry Low, No Iron

Do you think that covers everything? Is there anything I'm missing or that I have on the wrong side? Thanks for any help/advice! I'd love to orders these labels ASAP so I can be in compliance with all these laws!

I have been searching for a label example like this one. My question is: Does the fiber % cover the entire blanket since it is made from 2 different materials sew together or would the tag need to say :Front 100% cotton, back 100% polyester to be accurate?
