CPSIA Tracking label help

I make baby clothes memory quilts. What needs to be on my tracking label specifically? I found a website that said this:

1. Manufacturer or private labeler name;
2. Location and date of production of the product;
3. Detailed information on the manufacturing process, such as a batch or run number, or other identifying characteristics; and
4. Any other information to facilitate ascertaining the specific source of the product.

So mine would say:
1. WhirlyBirds
2. City, State or just USA? And how am I supposed to get labels with specific dates on them?
