Want to add a question/preference option in checkout -

HiHi! 

I'm Kailey, the Order Support Brat (and Order Fulfillment Wench) over at Creative Kink's Etsy store.   (Hey, don't knock the titles, the guests's LOVE them, and at in person events at B'nMs and vending, I get asked for autographs  ^.^  ... but only on my business cards with the title.   Thank god the owner is a photographer, so all of our business cards are model quality images of us -- ) 

So we've recently started sending out stickers printed with our business name and either the web address of our Etsy store, or the main site (we have a Shopify that carries the stuff that isn't 'Etsy-friendly' (a lot of our fan-base are heavier into BDSM than Etsy is comfortable with, which we try to respect) -- and it's working.   Sales are increasing, trackably (we note the orders "laced" in such a manner, and the increase in repeat sales is a double digit percentage increase, as well as a sizable increase of cross-site traffic and conversion.)   

But we're coming across an issue that our guests are sending us surprisingly kind feedback about --- cutomer preference.   It's something that's probably a bit more personal and specific to our lifestyle -- stickers are simple and cute enough for most businesses, right?    But in the lifestyle... people take their identity pretty seriously.   We have a GREAT reputation off-line, and on-line it holds over enough that no one has been offended (they laugh it off and find a friend with the right ideology, or toss it) -- but we still know it's uncomfortable for them, our guests, as part of the experience.   And it's a little awkward to find out we sent a "mostly generic attractive artistic" image to a lesbian couple that has a male in it, or a (Female dominant, male submissive) couple that has a women in (non-nude, tasteful) japanese rope art... etc.   They're all artwork one of our owners has the rights to -- but we'd really like to find a way to give them a choice at checkout, to identify themselves privately.  

Is there a way to add in, or an app to add in, a radio button for choices they can select for FemDom, Malesub, Lesbian, Asexual, TransPride, etc --- , or even just a short sentence above the "Notes" field in the checkout of "We Include Gifts From Time to Time - What is your Preference?" type of thing?  

Help?   Any ideas?   Preeeeeetttty please?

Why not create some cool stickers that are the business logo and website addresses to give out? Seems you are making it more difficult than it needs to be. If your customers have vastly different lifestyles then you have to find a way to be truly neutral with extras that are sent.

The only way on Etsy to do what you want is to have variations for each listing and they would have to choose  from "FemDom, Malesub, Lesbian, Asexual, TransPride, etc"

There was another post the other day, along the same subject.  They want to offer a free gift, and have the Buyer choose one.  I thought this was a bad road to start travelling, because some people can be picky, even with free stuff.

I actually do something very similar in my shop - I make my own hangtags with my info on one side, and the other side is various art that I buy as downloads on Etsy.  I have quite a collection, and it's a fun thing that I love to do - and I have a wide variety of vintage art that includes classical nudes, fantasy figures, and a variety of religious imagery from many cultures.  I think they are all beautiful and unique, (nothing that I wouldn't give my grandchildren, or that you may see in a museum) but realize any given recipient may be put off by some of them, for whatever reason.

My attitude towards this is - Meh!

It's free, I like them, if the Buyer doesn't, then toss it!

I sometimes look at buyer's profile and if there is a small inexpensive item in their favourites, that I happen to have, I will include it as a free gift.

I'm surprised by how often the recipient never mentions it or even worse, when they return their purchase, but keep the gift! 

I do it for my benefit, more than for theirs.

You could put a question in the personalization box within your products, and make it not mandatory to fill out. But it would probably stop some buyers from purchasing because it's just another step in the buying process, or some would misunderstand the reason behind the question.
