Some Info Customer Entered Didn't Show on Packing Slip

I had a customer order an etched wedding plate. Normally these have a large initial, wedding date, couple's first name and their last name. But on the packing slip it only gave the first name of Laura, date and didn't have the name Myles as the second half of the couple name and didn't show a last name. I contacted the customer twice via etsy messages and also twice via email asking if this was correct but got no response. Ship deadline arrived and I had no response. I shipped w the plated etched with the exact info that appeared. Customer contacted me after receiving the plate saying it should have had Lauara & Miles and the last name of Harmon. (Note: She's ordered from me twice before with no problems like this.)

She sent me a shot of what she got from Etsy on her end which has the complete correct info and I also asked her to provide the order info. It seems to be an Etsy issue as her info was correct but what I had to print out was not. So now I've agreed to do a corrected etched plate and ship it to her bc from what I can tell it wasn't either of our faults. Is there a way to have etsy reimburse the cost of the unetched plate which is $1.25 plus cost of having to ship again? I don't understand how this could have happened. This is for order [REMOVED]

You can't post orders here; it has confidential information and violates buyer's privacy.  As to your other issues; only sellers are here, we can't help you.

@SerendipityStudioOrg   You need to delete the personal names and the link to the order in that post above.  Also you will need to contact Etsy concerning what what showing on the packing slip.

Has the "buyers notes" been unticked from the customise options on the packing slip print box?

@SerendipityStudioOrg: There are several other threads in this forum about issues with personalized information not getting to sellers correctly. You might want to check some of those to see if you can get some help from someone who has gone through the same thing. And as mentioned before, the sharing of private information in the forums is against Community Policy, therefore you should edit your post.
