How are you registered with Minn? All information must match including all legal forms of name. That includes bank account. So do all forms of name match?
Does your shop reflect current addresses on all areas where this information is recorded? Does your driver ID have your current address if you moved at any time.
I don't see the normal line that Etsy adds so I wonder if you are running into the same problem as some other shops have with identity. Did Etsy recently get your information for the Inform Act?
I would check to make sure your llc information is still correct with your state. Also maybe check with IRS to see if any problem with your SSN.
