I want to offer a printable that the customer can buy either "as is" or add a name to it.
1. It looks like I can offer it in the same listing and they can choose whether or not to add the customization to it, is this correct?
2. If this is correct, what file do I upload? It makes me upload a file before I can publish it. I uploaded the generic version. Will the customer receive that even if they order customization?
3. Is there a different file I should upload?

If you are offering a digital personalised item which is different for each buyer, you can use the personalisation box to have the buyer provide their details (make it a must fill field). Then you can create an explanatory document that says their item will be created and sent to the buyer's email when ready, and upload that document to the listing so every customer ordering that listing will get that after payment. Then when the specific buyer's actual file is ready you can email it (the buyer's email is behind their Etsy user name on the order).
@ThatchedRoofCottage; Sounds like you want to make made-to-order digital orders. But you would still have to manually complete non-personalized orders if doing it within the same listing.
