Because we sell food products (our olive oil, for which we grow the olives), we have Health Dept regs we have to follow, one of which is inventory tracking, so I have a data base of all customers. I got an order yesterday, and the customer's name sounded familiar, so I went to Order History on Etsy and it showed only this order. When I went to log it into our inventory tracking, the name popped up, and as I found out, she's ordered several times (which is why I remembered the name).
This might not make a difference to some, but we always send a personal thank you note, and it makes a difference in the note whether they've ordered previously or are a new customer. This might also make a difference to people who offer incentives to repeat customers. It's not showing the correct order history.

Orders are linked but perhaps she is using a different email or different device which accounts for the orders not being linked. If she ordered as guest they are not linked then either.
Etsy will only flag an order as a repeat customer if the same account was used for each order. It's possible they previously checked out as a guest or used a different account.
@AyOOliveOil: We also do not depend on Etsy for maintaining our business records and have run into this, too. But it has always been as @AutumnHollowStudio stated, a purchase as a guest or using a different account.
You can search the customer's name on the orders page & it may pull up the same buyer using multiple Etsy accounts. I sometimes have the same buyers purchase with variations of their name or by using a spouse's name, multiple Etsy accounts, & guest purchases.
