I have a listing that was showing Etsy Bestseller last week - it quickly disappeared but the same thing happened last night. Does anyone know how often it refreshes? It seems to be gone as quickly as it came!

whatever sells best gets the badge, and what sells best can change very quickly.

So, don't attach any specific value to it, - it's also often called the "kiss of death", because it han happen that it slows down sales.

Why? Not everybody wants something that a lot of people have.

Etsy 'awards' the best seller badge to the listing that is currently selling the best ... in that category. Because the categories can be so broad, these competing items can be totally different. New products are coming online every minute of every day so there's no predicting if/when and for how long you get that BS badge. For clarity, BS could be short for best seller or 'something else' : )
