I need a custom printable!

I'm looking to have a custom printable label made about 2.5x3" in size. It's for my grandmother's 90th bday. I want it to be like the smuckers birthday announcements Willard Scott does along with her picture. I'd like to keep it at $15 or less.

I believe there are templates online but I don't have a program to edit it.
