In need of Customer Service Help!

I am in need of some advice, suggestions, etc etc.

On June 22, a customer placed an order for a custom made item. Upon check out they did NOT leave me a note letting me know what size they need the item crafted in. (it's one that requires a size) It is now July 26th.

I have since sent 4 Etsy Convos to them, asking what size they need their order in, PLUS, I sent 3 regular emails asking the same thing, in case the etsy ones were going to their spam folder.

I HATE having orders just sitting and not getting filled this long. I usually have items completed and shipped within 3-4 days, so this is Driving me nuts, that I can't even START her order, and she won't respond to me. Payment IS received so that is not an issue.

So here is my dilemma, what should I do? She's paid for the order, but won't respond to correspondence asking for the size she needed.
Should I 1. create the order in the size that was in the picture, or
2. Create HALF the order that didn't require a size requirement, mail it with a note saying that I can't complete her order until she responds to my emails concerning the size needed
or
3. Simply REFUND her money and cancel the order completely??

what would be the best course? I don't want to upset the customer, but at the same time, I just don't know what to do, and I don't want to take her money if I'm not able to give her the product she wanted.
