Samples of your CPSIA and FTC labels

Does anyone have a copy or picture of what their CPSIA labels look like? And/or your FTC labels? I have been looking at a lot of different items in retail stores to compare them to my labels and I can't even find consistancy anywhere. I made some of my own labels for items that I have made/sold so far and I am starting to doubt what I had even after all the research I did beforehand. Anyway, I thought some examples from other crafters might help or even put my mind at ease. Thanks everyone!

I made my own labels, and they came out great. I used a printable cotton (by Avery) that has fusible backing, which I didn't realize at the time I bought it. So, I fused the labels to 1/2 inch twill tape.

My labels have four lines:
Made by my name
my Etsy address
my city, state and country
the date

This worked perfectly, except that it looks horrible on my little test Gnome. The tag is half as long as the toy, it just gets in the way, and there really is no logical place to sew it on anyway. I feel like I've done all I can to get tags on my items, it just doesn't work. I'm going to have to go back to hang tags. I plan to have my logo, shop name and address on one side, and the made by me, location, and date bit on the other with a "please keep this for your records" line. I know they don't prefer it that way, but I really tried everything else.

The labels themselves are perfect, really, and making them wasn't difficult. I can take pics when I get home from day-job if you want to see. The labels are fine, I have to admit that my items are just too small for them.

Silly question: does the date need to be DD/MM/YYYY or would MM/YYYY suffice? I'd hate to have to make a new label (wasting time and fabric) every time I finish something. But I could have a monthly label batch. Or I suppose I could just hand write the date on...

Hit post before I was done!

I do believe DD/YYYY is enough (can't find a link to reference this right now). Some people are writing their dates in, but there is some question about the ink used to do this, since ball point pen ink is not on the exempt from testing list. I'd personally try to avoid that if you can.

Not according to the exempt list.

(it's on page 7 of the guide, page 8 of the document)

Many are doing it anyway. I wonder if it depends on the ink and that's why it's not on the list. More likely they didn't look into it because they don't expect a finished product to use it.

I'm glad I found this, almost everything I make is out of different materials, I will need labels I can write on. What about non toxic fabric paint pens??

Paint pens need to meet the even lower lead level of 90PPM than a regular pen which would has to be lower than the 300PPM level. If you are able to get the GCC or test results from pens manufacturer that reports the lead levels in PPM as long as it's below the allowable limits then yoou can use it to mark your tags. If the manufacturer says that they meet another agency's regulations have them provide you with the requirements to see if it is the same requirements as the CPSIA needs to have met.

MeowWow, I was just reading in the FTC handbook on labeling and they do have a clause for unknown fiber content and unknown country of origin (as in fabrics and yarn we bought before we knew we needed to keep records).
It says we can put on the label: Made of clippings of unknown fiber content/ 100% undknown fibers--rags/All undetermined fibers--textile byproducts/100% miscellaneous pieces of undertmined fiber content/Secondhand materials--fiber content unknown

These are all examples of how we can word the unknown on our labels. I found this on page 17 of "Threading your way through the labeling requirements under the textile and wool acts" handbook from the FTC.

I fill in the date when I sell the item. But don't always remember to attach the label. Also not the easiest to sew on a knitted blanket

If I'm sewing an applique onto a onesie and both are "made in China" and 100% cotton, do I need to add an additional materials label or just the CPSIA label?

To clarify:
CPSIA -- business name, contact info, DOM
FTC -- materials, country of origin

Is that it?
