Well, I decided to call and have a chat with the woman who is processing and sending out the letters. She was very nice when I explained that we are not their definition of small businesses. We are one woman operations in many cases. And that many do not have domains/web sites. I have a website but I hadn't set up my webmail on it. So I used my personal Fayemalonedesigns web address. Well, they need to go directly to a domain, as they are trying to fit us little guys into the big guy mold. She agreed that there needed to be some revisions. We chatted some more, and I explained about our etsy teams, and she cut and pasted me a quick format for you all to use.
I went ahead and set up my web site email, and she deleted my first registration and I'm going to reregister, using my domain site only.
But if you use your personal email, or other email that they can not verify that you are a direct contact you will get a letter, Do not be concerned, just fill out the information simplifed below and send it to her. It does not have to be on company letterhead.

Per our conversation here is the easier format for the small companies to follow. First they need to register, they will receive a validation letter request from us, and then they can follow-up with a the information I have below:

And a statement with the following:
I certify that (name of your company) is a manufacturer, importer or private labeler of a consumer product or other substance over which the CPSC has jurisdiction and that (name of the person) is authorized to receive and comment on Reports on behalf of (name of your company).
