Replacement/refund terms

I took a look at the post about the wording of refund policies and it got me thinking about replacement policies.

For those who do replacements, how do you handle them and word your policies? Basically my question is about shipping. For a refund I would have them ship the damage items back to me and refund the original price including shipping, but they would be responsible for shipping it back to me to show it was damaged.

Now if I offered a replacement who is responsible for what? They would still have to ship the damaged product back to me before I sent a replacement. Would I have them pay for the return shipping and then just not charge to ship the new piece or should I handle both the return ship fee and the new ship fee?

If an item arrived damaged I would cover all shipping fees (both to return to me and resend a new item). It's how I would expect to be treated as a buyer.
