A quick question - please help

I've posted in this before and I'm just looking for some more feedback before I contact my customer.

I had a customer make a purchase and it didn't apply the coupon code which she said she used - it was a 40% discount code. Is there any way to verify if she really used it? Or if it's a bug. The code has been active in my shop for almost a week now.

Someone said that she didn't click apply. Would that be the only thing to cause the coupon not to be applied?

She may have not clicked apply; I would refund the difference personally. I have not had any issue with people applying coupon codes but as a buyer too I have forgotten before.
