Can't locate Customs Form for International Order

I had my first international order yesterday and since it's been so long the process has changed.  Does anyone know how I get a customs form?

I printed out my shipping label and packing slip, but there was no customs form.  Does that mean I don't need one?  It's small but going to Malaysia so I'd be surprised if I don't need one.

I guess it is all in the code on the computer which in turn is on your slip that you printed out as shipping label.  I make sure I insure the package for whatever it is worth in case they lose it.  TamiJ
