Calculated shipping work around

I have been trying to figure out how to use Calculated shipping for a combination of circumstances.
I sell some items that I can only ship via UPS (Hazardous) and only in the contiguous US states. I need to charge a flat rate of $18 for those items and I would like to charge, and an additional $1 for each of those (specific items). I would also like to add "calculated" shipping on TOP on that for additional items my customers can purchase. Would a "handling" charge do that for me? I had an order that was placed and it took the $1 (each add'l item) yesterday and no $18 charge. I changed the add'l item charge to $18 but now if my customers order two of those items they will be charged $36 instead of $19. I could change the weight and size of the box to something absurd to force Etsy to calculate two boxes, but that still won't guarantee that. If anyone can help me, I would really appreciate it. Brain cannot seem to figure out a way to make this work.

The way the shipping calculator works is it takes all the calculated listings first and then adds in the "with another item" price for non-calculated items. I don't think there's any way around just putting the full price in the with another item box for those items that can't ship by USPS. The handling charge is added to each item when using calculated shipping, so that won't help for the non-calculated ones.
